Annual Student Health History needs updated at the beginning of every year or when enrolling in the district.
Medication (Physician Authorization) is used for ANY medication that needs to be administered during school hours. Prescription, over-the-counter, long term, and short term medications must have this form completed and signed by a physician and a parent.
Private Physical Form is for any student who wishes to receive their physical at their own private physician's office.
Private Dental Form is for any student that wishes to have their dental exam performed at their own dental office.
2017-18 Sports Physical Form is needed for any student interested in trying out or playing CASD Athletics. This form changes year to year and is only valid after June 1st 2017.
